The Minnesota Department of Human Services (DHS) has awarded $2.4 million in grants to community organizations across the state to help aging Minnesotans live independently and remain in their own homes as long as possible.
The grants will provide seed money to develop services that improve chronic disease management, link formal and informal long-term care services and promote independence for seniors, according to a news release from DHS.
The money comes from the Community Service and Community Services Development Grant Program, which has funded 300 Minnesota projects since 2001.
Twenty programs and organizations received the grants, including seven in the Twin Cities metro area.
